Your enrollment at East Texas Baptist University is NOT considered withdrawn until you have completed this form, met with Academic Success, and received confirmation of the withdrawal from the Office of the Registrar. The student is responsible for making sure that this form is filled out accurately and completely. A student who desires to withdraw, either from one course, multiple courses, or from the University, must officially withdraw in order to protect his/her academic record. Provisions for withdrawal are outlined in the University Catalog under the section entitled, “Withdrawal from a Course or the University.”
