TRANSFERS: Transfer status is given to any student who has attended college after graduating from high school or obtaining a GED. Students who have attended ANY college must have official transcripts from each college sent to the ETBU Admissions Office.

Authorization to Apply Federal Title IV Program Funds:

Students who receive federal Title IV funds (Pell, SEOG, TEACH, PLUS, Perkins and Direct Loans) as part of their financial aid package are required to complete a Title IV Authorization form. This authorization will allow any excess Federal Title IV program funds after tuition, fees, and room and board, to be applied to other allowable educationally related charges (books, supplies, insurance, library fines, parking tickets, late fees, and others) while attending ETBU. This agreement will remain in effect for your entire academic career at ETBU. You may, at any time, change your authorization by providing written notification to the ETBU Financial Aid Office.

I consent to the release of information concerning my academic and/or financial status to scholarship donors. I understand that if any information on this form is found to be inaccurate or incomplete, it will be sufficient cause for rejection or dismissal. I agree that if my student account, for any semester enrolled, becomes delinquent and all methods of collection have been exhausted by the University, my account will be turned over to a third party collector and I will be responsible for all collection and legal fees.
