This form should be completed with the student’s advisor once the student has been admitted to the university and is ready to enroll in classes. Filling out this form will create an accurate degree audit. This form is for initial declaration of degree and specialization or to change a specialization within a specified degree.
Please do not use this form to change a student from one graduate program to another. Changing graduate programs must be done through the Office of Graduate Admissions.
